PERMANENT SUPPORTIVE HOUSING

Dignity Village consist of scattered site – integrated permanent supportive housing, that targets individuals with high acuity needs, including individuals who has a history of chronic homelessness, have a diagnosis of severe mental illness, substance use or a history of incarceration. 

Our Permanent Supportive Housing program provides, long term housing, and on and off-site support services, that assist residents in achieving goals of sustainability, independence and recovery.

Impact

Dignity Village residents get much more than a place to live. They learn new skills, develop reliable sources of income, and often become contributing members of their community.

of clients in our permanent supportive housing programs continued to be stably housed one year after they entered our program

of our residents remained free from relapses from illegal substances, and stablized with their mental illness, compliant with their treatment.

of clients who exited our program maintained stable housing, maintained their income to remained housed, and maintain their recovery.

HOUSING ELIGIBILITY:

  • Must be referred from a partnering agency.
  • Currently chronically homeless.
  • Must be 18 –65 years of age, single male or female
  • Must be diagnosed with severe mental illness or substance abuse or dual diagnosis
  • Must agree to attend any and all community groups that support one mental illness and/or substance abuse.
  • Must self-administer medications
  • No history of violence or sexual abuse
  • Must adhere to all community housing rules and regulations.
