Primary and Behavioral Health Integration

Description of Services:

The CYH Project serves as the “Health Home” for those who receive services at Integrated Healthcare Systems by offering primary health services and wellness activities. This service structure provides follow-up medical care coordination, lab testing, and medical screenings.

The program upholds the values of educated choice-making, recovery-focused and person-centered therapy, and integration of evidence-based practices into standard medical practice. A person-centered, integrated approach to the medical care of people with behavioral health illnesses is the core of the CYH Project’s service delivery.

Our Goals:

Increase the provision of a coordinated range of medical, self-improvement, interpersonal, supportive, and rehabilitated programs that are tailored to the wants and preferences of the individual.
To enhance the physical well-being of individuals with severe mental illness who run the risk of having behavioral health and primary care issues co-occur.
To support those who are actively participating in their health and wellness.
To aid with the coordination of behavioral, specialist, and general healthcare treatments.
To improve individual and family participation in the planning process for rehabilitation.

Supports & Services:

Primary Medical Care – A primary care team will be available 5 days per week. Focus is on assessing, diagnosing and reducing the symptoms of medical issues, and making referrals to specialty care and wellness activities.
Care Coordination – Every effort will be made to coordinate our member medical appointment with ongoing psychiatric, rehab, and case management services, as well as assisting individuals with linkage with specialty care services in the community when needed.
Peer Support Wellness – The peer support specialist assists persons with their wellness by providing individual coaching and support to implement change strategies to improve healthy living habits. Smoking cessation, healthy eating habits, exercise, and healthy living groups will be scheduled throughout the year for participants in the program.
Integrated Services – Primary services will be coordinated with outpatient services, and staff will consult to assure integration of the medical, and wellness activities with the behavioral health services.

SAME-DAY APPOINTMENTS

Do you need care now? IHCS offers walk-in service through same-day appointments, allowing you to seek primary care treatment as soon as possible.

Our providers believe in providing primary care which allows a member to find help when most needed. Whether you’re facing an unexpected illness or need a follow-up visit, you can schedule an appointment usually the same day. This convenient service means you can avoid emergency room visits, long waits, and unfamiliar care centers.

PRIMARY HEALTH

ADULT HEALTH CARE

IHCS medical care services include routine adult and adolescent health care services, annual examinations, vaccinations, wellness screenings, chronic disease management and patient education.

WOMEN’S HEALTH CARE

IHCS is proud to offer a variety of women’s health services to our community. Our medical providers offer safe care for women while empowering them with the education they need to take control of their health. Early detection and preventive care are crucial to women’s overall health and wellness, which is why we provide annual exams, disease screenings, family planning, and much more.

PEDIATRIC HEALTH CARE

IHCS pediatric care includes well-child care; pediatric hearing, vaccinations, routine health screenings, sports physicals and adolescent care. IHCS age limit for pediatric care is 12 years and up.

CHRONIC HEALTH MANAGEMENT

IHCS will partner with you to screen for chronic illness prevention

If you are diagnosed with a chronic illness, we will partner with you to diagnose, treat, and manage your chronic disease. A member-specific treatment plan will be created using verified and trusted methods to help you reach the best level of health possible.

IHCS medical providers specialize in the management of the following chronic illnesses:

Hyperlipidemia (High Cholesterol)

Hypertension (High Blood Pressure)

Diabetes

STD TESTING & TREATMENT

Our Early Detection Intervention program assists in alleviating barriers to early Sexually Transmitted Diseases and HEP C detection – A quick and easy way to KNOW WHAT YOU NEED TO KNOW ABOUT YOUR BODY, WHEN YOU NEED TO KNOW.

Full Panel Sexually Transmitted Diseases include:

Gonorrhea
Chlamydia
Syphilis
Herpes
Hepatitis C
HIV

TELEMEDICINE

IHCS offers telemedicine for our patients. It’s as easy as scheduling time with your provider via video or phone. Safe and private, this consultation brings IHCS to you.

For members who lack mobility or transportation, telemedicine is a particularly convenient option since you can still schedule primary care visits with your physician.

Connect with your physician or Care Team member via video camera on your smart phone or home computer. You may also have a caregiver, friend or family join the visit just like an in-person office visit. Your Care Team and your physician will develop a plan, providing the same high-quality care as an office visit
