Nestled within the heart of Palm Beach County, Dignity Village by Integrated Healthcare Systems (IHCS) stands as a beacon of hope and transformation for individuals facing the harsh realities of homelessness. This initiative offers permanent supportive housing solutions tailored to meet the high acuity needs of those who have experienced chronic homelessness, severe mental illness, substance use disorders, or a history of incarceration. Through a blend of long-term housing and comprehensive support services, Dignity Village not only provides a safe haven but also empowers residents to rebuild their lives, fostering sustainability, independence, and recovery. 

A Comprehensive Approach to Permanent Supportive Housing 

Dignity Village operates as a scattered site-integrated permanent supportive housing program, targeting individuals with the most significant needs. This approach ensures that residents receive personalized care and support tailored to their unique circumstances. The program is designed to offer much more than just a place to live; it provides a nurturing environment where residents can learn new skills, develop reliable sources of income, and ultimately become contributing members of their community. 

The success of Dignity Village is evident in its impressive outcomes. A remarkable 97% of clients in the permanent supportive housing programs remain stably housed one year after entering the program. Additionally, 93% of residents have remained free from relapses into illegal substance use and have stabilized their mental health, complying with their treatment plans. Furthermore, 70% of clients who exit the program maintain stable housing, sustain their income, and continue their recovery journey, illustrating the program’s effectiveness in fostering long-term stability and independence. 

Amenities and Support Services 

Dignity Village offers a range of amenities and support services designed to create a supportive and empowering environment for residents. These include: 

  • Single/Shared Fully Furnished Rooms: Comfortable living spaces that provide a sense of home and stability. 
  • Transportation: Assistance with transportation ensures residents can access essential services and opportunities. 
  • Case Management: Personalized support from case managers helps residents navigate their recovery and achieve their goals. 
  • On-site Support Staff: Dedicated staff members are available to provide immediate assistance and support. 

Housing Eligibility 

To ensure that Dignity Village serves those most in need, the following eligibility criteria apply: 

  • Must be referred from a partnering agency. 
  • Currently chronically homeless. 
  • Aged 18–65 years, single male or female. 
  • Diagnosed with severe mental illness, substance abuse, or a dual diagnosis. 
  • Must agree to attend community support groups related to mental illness and/or substance abuse. 
  • Must self-administer medications. 
  • No history of violence or sexual abuse. 
  • Must adhere to all community housing rules and regulations. 

Specialized Housing Programs 

IHCS’s commitment to providing comprehensive homelessness support services extends beyond Dignity Village. The organization offers several specialized housing programs, each designed to meet the unique needs of different populations. 

  • Bella House 

Bella House is a long-term supportive housing program specifically for previously chronically homeless women with severe mental illness, substance use disorder, and co-occurring disorders. The program includes on-site and off-site mental health treatment, substance use treatment, primary healthcare, case management, vocational and employment services, life skills training, money management, and other support services. Bella House provides a safe and supportive environment where women can rebuild their lives and achieve long-term stability. 

  • Exodus House – Re-Entry Program 

The “CHANGES” transitional housing re-entry program at Exodus House empowers formerly incarcerated men to advocate for social change and personal transformation. The program offers a wide range of services, including mental health and substance use counseling, basic life skills training, resume building, employment referrals, relationship building, support groups, dispute resolution, goal setting, and other supportive services. Exodus House helps individuals reintegrate into society and build a stable, productive future. 

  • Hope House 

Hope House is a long-term supportive housing program for previously chronically homeless men with severe mental illness, substance use disorder, and co-occurring disorders. Like Bella House, Hope House offers a comprehensive range of services, including mental health and substance use treatment, primary healthcare, case management, vocational and employment services, life skills training, money management, and other support services. Hope House provides a supportive environment where men can work towards recovery and independence. 

  • Phoenix Place I & II 

Phoenix Place I and II are independent housing programs for men and women, respectively, who have mental illness or substance use diagnoses but have maintained recovery for over 12 months. Located in West Palm Beach, these programs are designed for individuals who have full-time or part-time employment and strong coping and life skills. Each resident has an assigned peer specialist as their mentor and a case manager to provide ongoing support. Phoenix Place I & II offer a supportive community where residents can continue their recovery journey and achieve greater independence.
