Homeless Outreach Transitional Stabilization & Wellness Program

What We Provide

The Host Wellness program engages unhoused individuals who do not readily access behavioral & primary healthcare services in the community.  Street medicine care is transitional by connecting unsheltered residents to comprehensive behavioral and physical health services. A critical component of the outreach model of care delivery is meeting the individual where they are physically and emotionally. The street outreach approach to care is predicated on the foundational idea that bringing services directly to the patient is the first step toward developing and maintaining a therapeutic relationship.

The HOST outreach teams consist of a Board Certified Nurse Practitioner, Medical Assistant, Mental Health Counselor and Peer Specialist. Street medicine outreach fills a gap in essential safety net services and is often the only medical care that many people facing homelessness are able or willing to access.

Services Rendered

The HOST team services include psychiatric evaluations, medications, medication refills, wellness examinations, COVID testing, COVID vaccines, flu vaccines, preventative lab work, STD evaluation, chronic disease management, basic wound care, case management, referral and linkage to detoxification, crisis unit stabilization, emergency housing and permanent supportive housing. 

The HOST & Wellness Program:

  • Reduces or eliminates barriers to behavioral and physical healthcare experienced by chronically homeless individuals
  • Enables behavioral healthcare problems to be detected and treated early.
  • Enables physical healthcare problems to be detected and managed.
  • Decreases admissions to crisis stablization units; and emergency room visits. 
  • Increases health promotion and prevention education amongst chronically homeless individuals. 
  • Promotes medication adherence through a partnership with Robalo Pharamacy.
